Clinics

General Tips:

  • Start with patient and caregiver together if feasible and okay with the patient (except in ED clinic).

  • If you order labs, make sure you cc your preceptor. If you manage results that come to your in basket, please cc your preceptor so they know what you did. If you are not sure, please fwd results to your preceptor with your proposal or your question.

  • Don't order point-of-care testing in Epic; verbally communicate with the medical assistant.

  • Make sure to LABEL specimens you collect including name, DOB, and site of swab.

  • Any sensitive exams (e.g. chest, GU) should be done with your preceptor.

Documentation:

  • Ask your preceptors if there are specific templates they recommend you use. 

  • Please try complete your notes within 24 hours.

  • When you are done with your note, please sign your note. Please do NOT sign the encounter.

Primary Care

We are a patient-centered medical home for teens ages 12-21yo. You will see patients for annual health maintenance, urgent care, and follow up care.

Tips:

  • At annual health maintenance visits, patients and caregivers are each asked to complete some a pre-visit questionnaire, and patients complete PHQ-9, GAD-7, CRAFFT screenings. These are on PAPER. Please review these and include when you present the patient to your preceptor. 

  • The PE form will be clipped to the door. Please complete and sign these. Please make a copy so we can scan into their medical record. 

  • Please use the new standardized Epic note templates. They are imperfect. Please ask your preceptor if they have any dot phrases they recommend adding to complete appropriate elements of visit.

Sexual and Menstrual Health

We see patients who are referred by their primary care providers for menstrual disorders, contraception,  LARC procedures, and other sexual health needs.  Because we are serving as consultants for these patients, your history and physicals should be more focused than they may be for a patient we see for primary care.

Gender & Sexuality Program

We provide specialized gender affirming care for children, adolescents, and young adults who are referred by the primary care providers.  You will observe a new intake visit with one of our social workers and see patients for medical follow up appointments.  Because we are serving as consultants for these patients, your history and physicals should be more focused than they may be for a patient we see for primary care.  â€‹

Eating Disorder Program

We provide multidisciplinary, family-centered care for children and adolescents with restrictive eating disorders in medical inpatient and outpatient settings.

Tips:

  • Spend time with dietitian and social worker to gain perspective on interdisciplinary care.
